Look Over My Package Options
Mini 1-month: Only recommended if you need to make just a couple of small tweaks to an otherwise healthy lifestyle
3-month: A great “in-between” option for those who have a little longer to travel on their path to wellness, want more accountability/encouragement, or those who have more complex health concerns
6-month: If you’re fully invested in your health/wellness but know you have some changes to make before you’re feeling your best, then this is the package for you.

Note: Before you can request an appointment, you’ll be asked to fill out a short assessment form. Your appointment request will need to be approved by Sam before you receive a confirmation.
If you purchase a package, you’ll receive a longer assessment form along with other PDFs to sign.
Initial Consultation
After you’ve purchased a package, we’ll meet for our 60-minute initial appointment, which we’ll spend going over your assessment form and discussing next steps/goals. These goals will be entirely unique from person to person, which is why we’re working together in the first place!
If there’s any lab work that I recommend, we’ll also discuss that during this consult. Any lab or supplement fees are extra (and are of course optional).
Customized ‘Next Steps’ Protocol
After our first appointment, I’ll send you your ‘Next Steps’ protocol. It will include the goals that we discussed during our appointment, plus any lab tests we discussed, along with a list of recipes, snack ideas, favorite cookbooks and other relevant lifestyle information.
Now it’s time to get to work on your goals!
Follow Through Together
As our time working together continues, we’ll have 30-minute follow up calls that will allow you to ask question, discuss any lab results, monitor progress and make new goals.
